Accountable Care Organization network

Ridgeview Community NetworkTM provides local, personalized care at one of the few independent health care systems in Minnesota. More than 300 physicians and nearly 2,000 medical and support staff deliver coordinated care locally within 30 primary care clinics and more than 150 specialty care clinics throughout a five-county metro area—including Carver, Hennepin, McLeod, Sibley and Wright counties—and beyond. The comprehensive regional network the breadth of services, innovation and technology of leading national networks right here within our own community.

Ridgeview Community Network includes Ridgeview Medical Center and Clinics, Ridgeview Sibley Medical Center, Lakeview Clinic, Ltd., Wayzata Children’s Clinic, Western OB/GYN, A Division of Ridgeview Clinics and OBGYN West (select providers only). In addition, Ridgeview Community Network partners include Abbott Northwestern Hospital, Hennepin County Medical Center and all of Children’s Hospitals and Clinics – including specialists located in Ridgeview’s Emergency & Urgent Care at Two Twelve Medical Center in Chaska. No referral is necessary to see a specialist within the network. In addition, all of Medica’s behavioral health, chiropractic and pharmacy providers are in network.

If you travel outside of Medica’s service area (Minnesota, North Dakota, South Dakota and western Wisconsin) and are in need of care, you have access to more than 715,000 physicians and care professionals and more than 5,400 hospitals in Medica’s nationwide network.

Important to note: All emergency care and urgent care services are considered in network.

Plan Highlights

Committed to providing you with the best possible experience while controlling costs, Ridgeview Community Network offers convenient tools and services to help you and your family members receive care and information quickly, including:

  • A Network Navigator—available to answer questions about the network and services, assist with choosing a primary care provider and facilitate communication between network primary and specialty clinics. In addition, the Navigator can provide information about community agencies, wellness services and more.
  • Same-day appointments—available at all primary care clinics and Saturday morning appointments at select clinics.
  • Urgent care—available 24 hours a day, 365 days a year at Two Twelve Medical Center in Chaska, Minn.; 6 a.m. to 9 p.m. seven days a week at Ridgeview Sibley Medical Center in Arlington, Minn.; and Monday through Friday, 8 a.m. to 8 p.m. and Saturday and Sunday, 8 a.m. to 5 p.m., at Ridgeview Delano Clinic in Delano, Minn.
  • Around-the-clock emergency care—available at Ridgeview Medical Center in Waconia, Minn., Ridgeview Sibley Medical Center and Two Twelve Medical Center.
  • Free “Meet and Greet a Ridgeview Community Network Primary Care Provider” visit—a 10-minute get-acquainted visit with a primary care provider during which you can discuss the provider’s practice philosophy and communication style to determine if you can work successfully together as a team.
  • Free local delivery of prescriptions from a Ridgeview Community Network partner pharmacy.
  • Online tools—allowing you to access online records, complete a comprehensive health assessment, receive personalized wellness recommendations, and find information about health events and classes, links to local resources, an extensive library of health information and more at health.ridgeviewmedical.org.
  • Medication Therapy Management (MTM) – care provided by pharmacists to help patients understand why they are taking their medicines and how their medicines are working, ensure medication safety to avoid drug interactions, find easier ways to take medicines, and address any patient concerns about medication including the cost and management of their medication.
  • 24/7 Nurseline to help answer questions and direct you to the appropriate level of care.
  • CareWire text messaging services —available from many clinics to remind patients of scheduled appointments and provide general information.
  • One-on-one personalized and group nutrition services.

How it Works

Ridgeview Community Network is an accountable care organization (ACO). In an ACO, groups of doctors, nurses and other health care providers work together with your health plan to provide coordinated care. You don’t need a referral when you see a provider in the Ridgeview Community Network.
